I came to Oak Hollow after having a stroke.  I’ve been here about 12 weeks and was a mess when I first arrived.  I had limited ability to move and limited ability to talk. The therapists are fantastic and the therapy department is outstanding.  They listen to you vent about your illness while you are doing physical things which has helped me. 

Stairs were a priority so I could go home, and I am doing great.  I am extremely pleased with my progress and will be going home soon.

Our Goal for Our Residents: 
Functional Independence & Quality of Life


Oak Hollow provides physical therapy, occupational therapy and speech therapy for our patients including those with clinically complex illnesses or injuries, bariatric patients, whether they will be at Oak Hollow for short-term or long-term care. 

We offer all modalities – ultrasound, electric stimulation, hot and cold packs, massage, and techniques for the reduction of swelling and inflammation.  We have new, state-of-the-art exercise equipment for our residents to use to build strength and stamina.

Whether the resident requires short-term or long-term care, the goal of our Rehab team is to reestablish and/or maintain function, quality of life and independence for activities of daily living (dressing, eating, and bathing).  A wellness program is available to our long-term residents that provides them with maintenance and exercise.  Our rehab staff continually looks to improve the endurance of our residents and their ability to ambulate independently whenever medically possible.

At Oak Hollow Nursing Center we have an Activities of Daily Living kitchen and bathroom where we assist with the retraining of skills residents will need upon return home.

Some medical conditions treated are

  • Reconditioning after illness, injury or surgery,
  • Orthopedic rehabilitation – fractures, joint replacements, and amputee rehab
  • Cerebral vascular accidents (stroke)
  • Subacute cardiac rehabilitation
  • Respiratory illness reconditioning
  • Rehabilitation for Neurological Conditions / Neuromuscular Disorders including Multiple Sclerosis and Parkinson’s

This tool assists us in our assessment and evaluation of patients.  It gives us an idea, based on each patient’s unique situation, of the level of improvement that might be obtained.  It gives us a starting point to set goals and their individual plan of care for rehabilitation.  We keep the patient and their family informed on how they’re doing , what is expected to happen, and what type of plans they need to make for discharge”.
